A template, not official guidance.

The requirement is set out in the EYFS statutory framework — use the version for childminders, and note a new one took effect on 1 September 2026. There is no required format for this, including this one: use whatever suits you.

1. Communication and language

Listening and attention, understanding, speaking. How do they let you know what they want? How much do they understand without gesture?

2. Physical development

Moving and handling, health and self-care. Walking, climbing, stairs, spoon and cup, toileting.

3. Personal, social and emotional development

Separating, playing near or with others, sense of self, coping when things go wrong.

4. Anything where progress is less than expected

Be specific and unalarming. Give an example and a date. If there is nothing, write that.

5. What I will do about it — and by when

The line most often left out, and the one the framework actually asks for. Include anyone else you are suggesting: health visitor, GP, speech and language therapy, Area SENCo.

6. What could help at home

Optional, and the part parents value most.

Before you hand it over

  • Written in plain English, with no age bands or jargonA parent outside early years must be able to read it
  • Any concern was raised in conversation firstNobody should meet it for the first time in writing
  • Every concern has an action and a review date next to it
  • Offered to share with the health visitor, with permission
  • Discussed in person, not just emailed
  • Copy kept, dated, with a note of the conversation
